Referring a Patient

The staff at EDGE Physical Therapy is dedicated to developing and maintaining strong working relationships with referring health care professionals and to making the referral process as efficient as possible.

Registration Forms

For your convenience, our new patient registration forms are available for downloading and completing before your first appointment. We will also need a copy of your insurance card upon arrival.

Accepted Insurance Plans

We accept all major insurance plans including Medicare. Cash based patients are accepted as well.

Billing Information

EDGE Physical Therapy automatically files all insurance claims for physical therapy services. All co-pays are due at the time of service. For your convenience, we accept cash, check, MasterCard or Visa.
